Attention Kindergarten Parents/Guardians:
Kindergarten students for the 2024-2025 school year will need updated vaccinations prior to the first day of school.
All Kindergarteners will require a total of:
5 DTaP (Diphtheria, Tetanus, acellular Pertussis)
4 Polio
2 MMR (Measles, Mumps, Rubella)
3 Hepatitis B
2 Varicella (Chicken Pox)
Vaccine information sheets are available in my office.
If immunizations are against your personal or religious beliefs, you must go to the following website and complete an exemption form.
https://cdphe.colorado.gov/vaccine-exemptions
Please follow the instructions and return the completed form to the front office at your child’s school. If your child cannot be immunized for medical reasons, a physician must sign a medical exemption.
Proof of required immunizations or exemptions should be provided prior to the first day of school or no later than September 1, 2024.
