Immunization Requirements for Students - 24/25 School Year

Hello Fort Lupton Bluedevil Families!

 

Welcome to the 2024-25 School year!

 

If you are the parent of a Preschooler, Kindergartener or a Middle Schooler, please review your child's immunization record as soon as possible,

 

Preschool students must be up-to-date on all immunizations or sign an exemption form every year or they will not be able to attend. 

 

In addition to their Preschool immunizations, Kindergarten students will need a booster to the following immunizations: Dtap, Polio, MMR, Varicella and Hepatitis B. Or, have a valid exemption form on file at the school. Exemptions need to be renewed every year.  

 

All Middle School students need a Tdap booster prior to their 6th grade year. Or, have a valid exemption form on file at the school. Exemptions need to be renewed every year.
